What is Healthy Forsyth?

Healthy Forsyth is a program funded by The Duke Endowment to address chronic health conditions such as diabetes and hypertension in zip codes 27101, 27105 & 27107. Healthy Forsyth houses a coalition comprised of representatives from Novant Health, Atrium Health Baptist Medical Center, Public Health officials, community clinics, nonprofit organizations, community partners, and community residents from zip codes 27101, 27105 & 27107.

What is the purpose/goal of Healthy Forsyth?

The goals of Healthy Forsyth are:

• Connecting community residents to resources to address their chronic health conditions.
• Remove system barriers to health care.
• Improve health outcomes of community residents located in 27101, 27105 & 27107.
• Expand programs and services to all of Forsyth County.

Healthy Forsyth partners with Una Bendicion to provide health information and activities for the food pantry.

Healthy Forsyth shares health information and support activities in two communities. Walk with Me includes providing information fairs as well as encouraging and leading walks in the neighborhood.
